The Hospice Team
Hospice care is provided through a team of professionals who work together to provide not only medical and comfort care to those in our care, but family support, spiritual support, social services as well. Our interdisciplinary team (or IDT) at Gateway Hospice includes:
Licensced Practicing Nurse
Family and Bereavement Support
This team meets every other week to discuss the Plan of Care and collaborate on how to best meet the needs of the patient and family.
